New Pharmacists
When completing the online application be sure to choose the Obtain By Method of “Application”. Requirements:
- NABP Application – NABP Website
- Graduates needing to take the NAPLEX must apply for the NAPLEX after creating your NABP e-Profile
- Graduates wanting to transfer NAPLEX scores into Idaho must apply for NABP Score Transfer
- Idaho PDMP Registration – Pharmacists must be registered with the Idaho Prescription Drug Monitoring Program. Registration can be completed at https://idaho.pmpaware.net New applicants must enter “pending” when prompted for your Idaho license/registration number.
- Certificate of Graduation – Certificate of Graduation Form must be completed by the school and have the school seal affixed
- Current government issued photo ID
- Fingerprint Card – For applicants 18 and older

Important Notes about testing:
- Authorization to Test (ATT) – After approval of a pharmacist application and graduation, verification is sent to NABP. After which the applicant will receive an email from Person Vue.
- Test Score Results – The Idaho Board of Pharmacy will receive test scores approximately 3-4 business days after the exam has been completed. Applicants are to check their scores via the NABP website. Do not call the Board office for results.
Pharmacist Reciprocity and License Transfer

Pharmacist Reinstatements
When completing the online application be sure to choose the Obtain By Method of “Reinstatement”. Requirements:
- Continuing Education – Proof of 30 hours of continuing education credits. Credits must have been earned within the 24 months prior to the date the reinstatement application was submitted
- Last 2 (two) Years of Pharmacy Employment – Must provide document showing the following:
- Business name to include full address & Phone number
- From and To Employment dates
- Contact person name to include Title & Phone Number
- Idaho PDMP Registration – Pharmacists must be registered with the Idaho Prescription Drug Monitoring Program. Registration can be completed at https://idaho.pmpaware.net
- Current government issued photo ID
- Fingerprint Card – For applicants 18 and older

- The following may apply to Pharmacists who have not worked in the pharmacy field for several years:
- 40 hours of internship for each year out of the pharmacy field
- NABP testing via the NAPLEX
- Completion of additional CPE hours
- Appearance before the Board
- Other requirements determined necessary to acquire or demonstrate professional competency
For each principle place of practice that a registrant will house or receive controlled substance medications will need a separate Idaho Controlled Substance Registration and Federal DEA Registration.
Idaho Practitioner Controlled Substance Registrations
Application Fee Exemption – When either of the exemptions below are applicable while submitting an Idaho Practitioner Controlled Substance application, select the Obtained by method “Application Fee Exempt”
- DEA Exemption – Persons exempt pursuant to federal law from fee requirements applicable to controlled substance registrations issued by the DEA are also exempt from fees applicable to controlled substance registrations issued by the Board
- Military Exemption – Any licensee/registrant serving in the armed forces of the United States is exempt from the payment of license fees during any period of military service. If applicable, upload proof of military service that includes the date active duty will cease

- Idaho Prescription Monitoring Drug Program Registration (PDMP)- Applicants must initiate an online registration with PDMP (Not applicable for Veterinary and CET registrants).

- As stated in Idaho law IDAPA.24.36.01.224.02.a., failure to obtain a federal DEA registration for any reason within 45 days of the issuance of the Idaho Practitioner Controlled Substance Registration will result in automatic cancellation. Issued registrants, refer to the email Notification of Activation for instructions on how to submit the associated federal DEA registration.

Practice Address Changes – Only for Address Changes Within Idaho
Per IDAPA Rule 24.36.01.501 Reporting Individual Information Changes. Practice address changes must be reported to the Board of Pharmacy within 10 business days of the date of change.

- After the updated DEA has been issued, save as an Adobe document, then log into your online e-Gov account via our website https://bop.idaho.gov/
- From the Menu, select License Update and Click on Continue on the active registration record.
- To upload the updated Federal DEA Registration, from the Menu select Attach Document/select Federal DEA Registration for the Type of document/click on Upload Document
- Once the document has been uploaded click on Attest and then Finish.
- Send an Email to BOP-info@dopl.idaho.gov to notify Licensing that the document has been uploaded so your practice address change can be completed in your record.
Idaho Researcher Controlled Substance Registrations

Options for obtaining a Fingerprint Card
Print a copy of a fingerprint card
Option 2 – Walk-In:
Contact local law enforcement agency, or fingerprinting company. If the agency/company does fingerprint services, they may also have fingerprint cards available.
Option 3 – By Mail:
Send an email to bop-info@dopl.idaho.gov, to have a Fingerprint Packet mailed to the applicant. Include the full address with a full name, mailing address, and application type.
All applicants, no matter what option chosen above must either deliver to our or mail completed fingerprint card to our office. Scanned or faxed copies are never accepted. Do not bend or fold the card.

- This new system is open all year and includes the ability to complete new applications, update personal demographics, licensing information, upload documents, and complete your annual renewal process.
