If you want to reinstate an expired license, please email our licensing staff at HP-Licensing@dopl.idaho.gov.

No practice is permitted prior to receipt of a license number.

Applicants are advised not to enter irrevocable contracts, purchase or sales agreements, on the assumption that licensure will be granted.

Applications must be on current forms provided by the Board or they will not be accepted. Please review State Licensure Law and Rules regarding the requirements for application and licensure prior to submitting an application.
