Practitioner Controlled Substance Registration

The Idaho Controlled Substance Registration (CSR) is required for practitioners who will prescribe, administer, order or store controlled substances.  Per Idaho law, once the Idaho CSR is issued, an associated DEA must be obtained for that Idaho address within 45 days, either as a new DEA or by moving an existing DEA to the Idaho address. 

Once issued a primary Idaho CSR, any additional Idaho practice address requires a separate Idaho CSR and associated DEA only if the practitioner will be administering, ordering or storing controlled substances at the additional address.

Application Requirements & Required Documents

  • Valid Idaho practice address – This must be a dedicated medical practice office or Idaho residential address. If using an Idaho residential address, we recommend contacting the DEA as that agency may have different criteria for residences.
  • Idaho Professional License
  • Initiate registration with the Idaho Prescription Drug Monitoring Program (PDMP) – Registration can be completed at https://idaho.pmpaware.net. You may enter “Pending” when prompted to enter your Idaho license/registration number.  PDMP registration is not required for Veterinarians or CET’s.

Application Fee Exemption 

  • DEA Exemption – Persons exempt pursuant to federal law from fee requirements applicable to controlled substance registrations issued by the DEA are also exempt from fees applicable to controlled substance registrations issued by the Board
  • Military Exemption – Any licensee/registrant serving in the armed forces of the United States is exempt from the payment of license fees during any period of military service. If applicable, upload proof of military service that includes the date active duty will cease

Application Fee Exemption – When either of the exemptions below are applicable while submitting an Idaho Practitioner Controlled Substance application, select the Obtained by method “Application Fee Exempt”
