Idaho law requires the licensure of all persons who practice or offer to practice Denturitry in the State of Idaho. Licensure is available through both endorsement and examination.
Please note the application may be filled in on the screen when accessed, but must be printed, notarized, and mailed or hand-delivered to the office with payment. The forms cannot be submitted electronically or faxed.

For your license to remain valid, it must be renewed before the expiration date each year. You cannot practice on an expired license.
IF YOU HAVE NOT YET MET THE CONTINUING EDUCATION REQUIREMENT, DO NOT SUBMIT A RENEWAL INDICATING THAT YOU HAVE. Submitting a fraudulent application for licensure is grounds for discipline against your license
Please note: If you fail to renew your license before its expiration date a reinstatement fee will be required by Idaho Code 67-2614 and cannot be waived. In addition, you may be required to submit continuing education or other required documentation prior to the reinstatement of your license.
Any licensed denturist who operates a denturist lab or clinic, or who conducts business as a denturist, must register their business location with the board annually (See Rule 475). The form for Denture Clinics must be completed and submitted to the Board within ten (10) days of opening for business. The form must also be submitted for any change in either location, identity of the principal denturist, or of any denturist employees. If you operate or conduct business at more than one business location, please copy and complete this form for each business location.
